Delaware Valley Community Health leverages RPM to support community-wide hypertension initiative

Delaware Valley Community Health (DVCH) found new ways to support patients and track important clinical data points. They launched a community health outreach program — Road to Controlled Hypertension — to improve provider engagement, implement technology, and increase controlled hypertension rates.
